2) Click SurveyMonkey Request Form.

3) Read the terms and conditions. If you agree, click I agree, request license for SurveyMonkey.

OUTCOME

You will receive a confirmation email for your request. The software licensing team at Technology Support will contact you when the process is completed. You can then create your account.
